Identify the Risks

Business continuity practices applied to Earth based companies’ saves money, jobs and perhaps lives. Disaster recovery practices applied to Earth based companies’ means a return to the work place, eventually.

Business continuity practices applied to an interplanetary or lunar colony saves lives. Disaster recovery practices applied to interplanetary or lunar colonies means you have failed, probably catastrophically, and mean a return to the work place will probably never happen.

The differences are obvious but the paths to the successful application of the best practices in both business continuity planning and disaster recovery are the same on Earth as they are in space. 

Redundancy is the first principle of disaster recovery and is critical to any well thought out continuity plan to prevent or mitigate mission failure. This concept is a well established practice in any space or aviation industry but it is what you failed to plan for that will undoubtedly ruin your day, and what you failed to recognize or chose to ignore would probably be a fatal flaw in any mission.

The Space Shuttle Challenger disaster was a fatal incident in the United States' space program that occurred on January 28, 1986, when the Space Shuttle Challenger broke apart 73 seconds into its flight, killing all seven crew members aboard. 

A special commission appointed by United States President Ronald Reagan to investigate the accident found that NASA's organizational culture and decision-making processes had been key contributing factors to the accident, with the agency violating its own safety rules.[1] 

This means that something other than mission safety took precedence. This means that although peoples lives were on the line, a conscious decision was made that put those lives in jeopardy.

These two statements are not a condemnation of those in authority on that day but an illustration how ignoring or failing to recognize a specific risk can cause mission failure.
